Medically reviewed by Dr. Bijan Afar, DDS, MS — Periodontist & Oral Implantologist

Whether it's a damaged tooth that can't be saved or wisdom teeth that need to come out, we perform extractions with care, and a focus on preserving the bone for future implants or restorations.

Most extractions are quick, comfortable, and followed by detailed aftercare guidance.

When Is a Tooth Extraction Necessary?

We always try to save a natural tooth when it is reasonable to do so. Extraction is considered when a tooth cannot be restored or is causing harm.

  • Severe decay or fracture that cannot be repaired
  • Advanced gum disease that has left a tooth loose
  • An infection that cannot be resolved with a root canal
  • Wisdom teeth that are impacted, infected, or damaging nearby teeth
  • Teeth that need to be removed before orthodontic treatment or dentures

Simple vs. Surgical Extraction

A simple extraction removes a tooth that is visible above the gumline. A surgical extraction is used for teeth that are broken at the gumline, have curved or complex roots, or are impacted, and it may involve a small incision in the gum. We tell you which one to expect, and we discuss comfort and sedation options beforehand.

Preparing for Your Visit

  • Tell us about your medical history, medications, and allergies, including blood thinners.
  • Ask about sedation options if you feel anxious, and arrange a ride if sedation will be used.
  • Plan for a quiet rest of the day and have soft foods on hand.

Aftercare and Healing

  • Bite gently on gauze to control bleeding, and replace it as directed.
  • Do not rinse forcefully, spit, use a straw, or smoke for the first day or as advised, since these can dislodge the blood clot that protects the socket.
  • Use a cold compress for swelling and eat soft foods at first.
  • Take pain relievers exactly as directed by your dentist or on the label.
  • Call us if you have heavy bleeding that will not stop, severe pain several days after the extraction, fever, or worsening swelling.

The surface usually heals within a couple of weeks, while the bone underneath continues to remodel for months.

Replacing a Tooth After Extraction

Once a tooth is gone, the surrounding bone can shrink and neighboring teeth can drift. When an implant or other replacement is planned, we use techniques that help preserve the bone at the time of extraction. Options include dental implants, dentures, and bridges, and we can discuss the right timing for your case.

Frequently Asked Questions

Does a tooth extraction hurt?
The area is numbed before the procedure, so you should feel pressure but not sharp pain. Sedation options are available for patients who are anxious. Some soreness is normal afterward and can be managed as directed by your dentist.
How long does it take to heal after an extraction?
The gum surface commonly heals within a couple of weeks, while the underlying bone remodels over a longer period. Healing varies with the tooth, the procedure, and your overall health.
What is a dry socket?
A dry socket can happen when the blood clot is lost from the extraction site early, which can cause increasing pain a few days after the procedure. If you notice this, call us so we can help.
Do I need to replace an extracted tooth?
Often, yes. A missing tooth can lead to bone loss and shifting of nearby teeth. We can discuss replacement options such as implants, bridges, or dentures.
Can I drive myself home after an extraction?
That depends on the type of anesthesia or sedation used. With local anesthesia alone, most patients can drive. If sedation is used, you will need someone to take you home.
