All-on-4 Dental Implants — A Full Arch in One Day

Medically reviewed by Dr. Bijan Afar, DDS, MS — Periodontist & Oral Implantologist

All-on-4 is the technique for patients missing most or all of their teeth in an arch. Four titanium implants are placed at specific angles that maximise bone contact and support a full arch of teeth. In most cases you leave the same day with a temporary fixed bridge already screwed in place. The permanent porcelain arch is fitted three to six months later, once the implants have fully integrated.

The technique was developed to solve two problems: patients who couldn't tolerate dentures anymore, and patients who lacked the bone volume for a conventional implant per tooth. By angling the two rear implants, All-on-4 uses the denser bone at the front of the jaw, often skipping the need for a sinus lift or bone graft. That means faster treatment, lower cost, and fewer surgical procedures.

Not every patient is a candidate. We do a full cone-beam CT scan at the consult to measure available bone and identify any anatomical factors that would rule it out. In some cases All-on-6 or a different plan is a better fit — we don't force cases into the technique because the marketing is good.

Dr. Afar has placed thousands of implants over 30 years, including full-arch cases as complex as any general practitioner will see. All-on-4 surgery is done under sedation, and you go home the same day with a temporary bridge and full aftercare instructions. Most patients report they underestimated how quickly they'd return to normal eating.

Frequently Asked Questions

Am I a candidate for All-on-4?
Most adults missing most of the teeth in an upper or lower arch, or facing that outcome, are candidates. The two things that matter most are adequate bone volume (which we measure with a 3D scan) and good general health for outpatient surgery. Uncontrolled diabetes, heavy smoking, and some medications can affect candidacy — we screen for all of that at the consult.
How much does All-on-4 cost in Los Angeles?
A single-arch All-on-4 typically runs $22,000–$32,000 in Los Angeles, including the four implants, the surgical procedure, the temporary bridge, and the final porcelain arch. Both arches together are usually $40,000–$55,000. We give a written all-inclusive quote after the 3D scan — no surprise fees mid-treatment. Insurance rarely covers full-arch implant work, but we offer CareCredit financing with plans up to 60 months.
How long does All-on-4 last?
The four titanium implant posts have a 95%+ 10-year success rate and often last for life. The porcelain arch on top gets replaced every 15–20 years as it wears. Regular check-ups every six months (which you'll come back for anyway to keep the surrounding tissue healthy) let us catch any issues years before they become urgent.
Will All-on-4 look and feel natural?
Almost universally, yes — patients report the transition from dentures to All-on-4 feels like going from renting to owning. The teeth are fixed, they don't move when you eat, and the bite is stable. Aesthetically the porcelain is shade-matched to look natural for your face and skin tone. We do a try-in with the final teeth before final cementation so you approve the shape and shade first.
How long is recovery?
Most patients take 3–4 days off work after surgery, though many return sooner. You'll eat soft foods for the first 2–3 weeks while the implants integrate. Some swelling and bruising is normal for the first week — we manage discomfort with prescription medication for the first 48 hours and ibuprofen thereafter.
